•    Did they see suspect fill out / sign the document
•    Did they see the suspect thumbprint the document
•    Can they / did they positively ID the suspect

Did anyone witness?

•    Who?
•    What can they testify to?

Do they have the document?

•    What company/bank issued the document
•    Who was document issued to
•    What name was used to endorse
•    What is the document number
•    What is the amount of the document
•    What is the account number
•    Did you contact the owner
•    Is the suspect known to the owner
•    Are the checks: Lost/Stolen/Computer Generated

Was the suspect alone?

•    If not, get a description
