Do not fill out the form electronically. The form must be printed exactly as it appears and not altered in any way. (Prior to submitting this application you must have taken and passed the written aptitude test.)

Once completed return it via mail or in person to the Wichita Sedgwick County Law Enforcement Trainng Center (WSCLETC) between 8 AM and 5 PM, Monday-Friday. If you are unable to print the questionnaire, you may come to the WSCLETC located at 2235 W. 37th St. N. or call (316) 660-3810 and pick up a copy.
